The Mobile Care Clinic

Weekdays: Monday’s alternating and shifting weeks around holiday and Wednesday’s all day (Paused temporarily)

This clinic provides walk-in primary health care, triaged and accessing the clinic through the SORCe engagement team. The clinic provides a nurse practitioner and a peer support worker. The team can help with physical health, including addiction medicine.

Hamper Outreach Program – The Alex Street Team & SORCe

Friday mornings

The Hamper Outreach Program is aimed at engaging people who might not be aware of, or regularly engage with SORCe and the programs offered here. This program is in its pilot phase launching officially in February 2023, and will evaluate opportunities for expansion with community partners over the coming months.

The Street Team

Walk-ins - Monday, Wednesdays, Thursdays and Fridays

The Alex Street Team aims to provide addiction outreach services to street-involved individuals engaged in substance use and/or facing mental health barriers. The Outreach Team utilizes a peer-support model by pairing staff with lived experience of homelessness, addiction, and recovery with a registered professional during outreach activities. The Peer Support Workers are staff members who work to reduce the barriers between service providers and those seeking support, and act as a beacon of hope and support by sharing their stories and experiences. Services provided by the team include the distribution of safer-use and harm reduction supplies, screening, assessment, brief intervention, case management, referrals to housing or other addiction services, identification support, and education to community services providers.

Services Offered:

  • Peer support, youth focused, depending on need/demand at SORCe and can also work with adults.

  • Addictions and mental health referrals and community resource navigation, NSQs.

  • Ability to conduct intakes for the Rapid Access Addictions Medicine (RAAM) clinic at The Alex. This would include a brief substance use assessment and clients would leave that intake with an appointment to see an addiction medicine physician at The Alex.
