Home Stay Family Prevention and Diversion–Trellis Home Stay Family Prevention and Diversion is able to provide financial support, short term case management and connection to community resources and activities for families experiencing financial instability, facing eviction, or who have recently become homeless. Services offered:

• • Help with rent arrears

• • Support accessing basic needs

• • System navigation

• • Financial literacy

• • Connection to community resources, programs and activities

Unity Youth Housing Outreach & Shelter Diversion

Unity works youth ages 16-24 who are at risk or currently Homeless, to permanently end their experience of homelessness. Through providing outreach case management, Unity diverts youth from accessing shelter and adult serving systems. Unity operates through evidence-based practices to create a system of natural supports so that youth will thrive and transition successfully to independence.  

Available Tuesdays 9am-4pm (closed 12-1), and Thursdays 1pm-4pm.
